Highly effective medications include monoclonal antibodies directed at CD20-expressing B cells (administred by injection or infusion, e.g. rituximab), natalizumab (an integrin) or alemtuzumab, which depletes white blood cells more broadly. Treatment of CIS with interferons decreases the chance of progressing to clinical MS. In general, aggressive treatment in the early stages of MS produces the best long-term outcomes. The relative effectiveness of different treatments is unclear, as most have only been compared to placebo or a small number of other therapies. There is high confidence that natalizumab, cladribine, or alemtuzumab are decreasing relapses over two years for people with RRMS. Natalizumab and interferon beta-1a (Rebif) may reduce relapses compared to both placebo and interferon beta-1a (Avonex) while Interferon beta-1b (Betaseron), glatiramer acetate, and mitoxantrone may also prevent relapses. Bethlem myopathy is predominantly an autosomal dominant myopathy, classified as a congenital form of limb-girdle muscular dystrophy. There are two types of Bethlem myopathy, based on which type of collagen is affected. Bethlem myopathy 1 (BTHLM1) is caused by a mutation in one of the three genes coding for type VI collagen. These include COL6A1, COL6A2, and COL6A3. It is typically autosomal dominant, though uncommonly can be autosomal recessive. Bethlem myopathy 2 (BTHLM2), formerly known as myopathic-type Ehlers–Danlos syndrome, is caused by a mutation on the COL12A1 gene coding for type XII collagen. It is autosomal dominant. In 2017, an international workshop proposed a redefined criteria and naming system for limb-girdle muscular dystrophies. === Surgery === The instability of joints, leading to subluxations and joint pain, often requires surgical intervention in people with EDS. Instability of almost all joints can occur, but appears most often in the lower and upper extremities, with the wrist, fingers, shoulder, knee, hip, and ankle being most common. Common surgical procedures are joint debridement, tendon replacements, capsulorrhaphy, and arthroplasty. After surgery, the degree of stabilization, pain reduction, and people's satisfaction can improve, but surgery does not guarantee an optimal result; affected people and surgeons report being dissatisfied with the results. == Production process == Stock cubes are made by mixing already-dry ingredients into a paste, including salt, fat, (meat/bone/vegetable) extract, etc. The ingredients are usually mixed in a container (batch mixing), left to mature, and then shaped into the cube form. Alternatively, they can be mixed directly into an extruder. The "extract" portion needs to be separately produced from the meat, bone, or whatever food is intended to be used in the reconstituted boullion (see meat extract). This is achieved by large-scale cooking similar to what is used to make pre-packaged broth, followed by an evaporation process to condense the product.
=== Part One === The first part of the film was created by Toronto-based David and Ian Purchase, the two of whom are collectively referred to as the Purchase Brothers. Before Escape from City 17, the Purchase Brothers had directed several commercials, including one for Coca-Cola. David contended that they worked as commercial directors in order to support their independent projects. Both being fans of the Half-Life series, the two decided to start Escape from City 17 as a way "to showcase and promote their talents further, and experiment with several post-production techniques they had developed." The two had a budget of Can$500; the computer equipment and software employed for the development of the film belonged to the Purchase Brothers from previous projects. According to David, the money was spent on the live-action elements of the film, saying that "the costumes, and used/broken airsoft guns made up the bulk of the budget." The two had no crew to support them, and were not paid for their work. Many of the elements of the film, such as the background, the Citadel, and the gunships, were extracted from Half-Life 2. The elements were then "graphically enhanced, and incorporated into the live action with a lot of complicated tracking and rotoscoping." The background used for the film's opening scene is the panorama of Riga. The "main set" of Part One was filmed at a trainyard with "active security." This made filming the short difficult, as the Purchase Brothers "tend not to get permits" to use the trainyard for the film.
